We
are pleased to introduce our new GuideStar application form. This form reflects
our commitment to the final expense market and our desire to increase your GuideStar
sales. It was designed by members of our Underwriting and Marketing Departments,
with substantial input from experienced final expense producers. We think the
new application will make it easier for you to use GuideStar products.
There are some things you need to know about the
new application. You will notice some differences. We want to explain them
to you.
- The new application (Form 2003 FEPA and state
variations) broadens the range of risks we insure. With each change we make
in our underwriting process, we will monitor our claims experience. Please
help us by using good judgment and good field underwriting practices. If
your customer has a condition not addressed by the application, but it appears
to be serious, call our Underwriting Department to discuss it. The risk
selection process begins with you.
- The new application was designed for greater
simplicity. We have removed the second insured from the application to create
more space on the form. Use one application form for each GuideStar applicant.
- The questions in the application have been reworded
for greater certainty of the outcome. To make this application function
for you and American Home Life, make sure that you answer all questions
accurately and completely.
- In 2002, we instituted a point-of-sale interview
service. This service has been well received by our representatives because
it is fast and efficient. When you call in to our office, you and your customer
will speak with one of our employees. Our interviewer will verify the answers
to questions on the new application. Call us to complete a point-of-sale
interview if the amount of coverage you are writing is $5,000 or more or
your applicant is 70 years old or more. The telephone number is 800-298-6020.
This service is available between the hours of 7:45 a.m. and 8:00 p.m. Central
Time.
- We recently introduced new authorization forms
(Forms U-80 and U-81) to comply with the requirements of HIPAA. The authorizations
are required if an Attending Physician's Statement (APS) is ordered. Because
very few physicians' statements are ordered in the underwriting of GuideStar
applications, Forms U-80 and U-81 are not required to be submitted with
the GuideStar application. On those rare occasions when an APS is required,
our Underwriting Department will ask you to obtain the authorizations.
- We are members of the Medical Information Bureau
(MIB). We make an MIB check on every final expense application. We will
take additional steps in the underwriting process if we obtain coded information
from the MIB.
The new GuideStar application is now approved for
use in your state. You may begin using it immediately. It replaces the old
application, so please destroy any of the old applications in your possession.
